I acknowledge the risks associated with playing competitive tennis, accept those risks voluntarily, and in consideration of my acceptance in the league, assume all risks for bodily injury, and waive all claims for injury.
I acknowledge that Gladiator Tennis is not responsible for the conduct of it’s members, and I release and hold harmless Gladiator Tennis and it’s employees and agents with respect to any misconduct or offensive actions by league players.
I also consent to the release of my email and phone number to other players in the league for the purposes of arranging practice and playing tennis matches.
I affirm that I am at least 18 years of age if playing singles for the adult league, and under 19 years old if playing for the high school league. (There is no age requirement if you are playing doubles or mixed doubles with an adult as your partner).
